ACEP Supports Physician Training Pathways for All Emergency Medicine Colleagues

The American College of Emergency Physicians (ACEP) affirms that MD (Doctor of Medicine) and DO (Doctor of Osteopathic Medicine) physicians stand on equal and level ground in the practice of emergency medicine.

While the pathways to becoming an emergency physician may differ slightly in training philosophy or structure, both MDs and DOs meet rigorous standards, undergo extensive clinical preparation and are held to the same high expectations in residency, board certification and clinical practice. In the emergency department, what matters most is competence, compassion and commitment to patient care — values that transcend degree type.

ACEP also recognizes and respects the contributions of International Medical Graduates (IMGs), who often face significant barriers in their pursuit of a U.S. residency position. These challenges include navigating a complex application process, overcoming visa and licensing hurdles and competing for a limited number of positions in an increasingly competitive Match environment. Despite these obstacles, many IMGs bring unique experiences, multilingual skills, cultural sensitivity and a deep dedication to medicine that enrich the fabric of our specialty.

Data from the Emergency Medicine In-Training Exam and ABEM Qualifying Exam results pertain to physicians who matched into emergency medicine 3 to 4 years earlier. Trainees entering the specialty in 2022 through 2024 — the period in which Match composition has notably shifted downward — have not yet taken these examinations and are not included in the current data. Any attribution of recent exam score trends to changes in Match composition is premature and not supported by the timeline of available results.

ACEP stands firmly behind all individuals who have chosen this just and noble profession. Emergency medicine thrives because of the diverse backgrounds, perspectives and talents of those who enter our field. Every MD, DO, and IMG is united by a shared mission to provide lifesaving care. We celebrate and support every colleague who answers the call to serve in emergency medicine.
