The rules for mapping medical chart documentation to the selection of code level of service are changing for the emergency department evaluation and management (E/M) codes, for the first time since 1995. The prior reliance on documented history, physical exam, and medical decision-making will end starting January 1, 2023, with a reliance on only medical decision-making (MDM) or time. Since time is not a descriptive element of ED E/M codes, that leaves MDM only starting in 2023.
ACEP has been actively involved in the development of these new documentation guidelines through its work in the CPT Editorial Panel process and has insight into why and how these changes were made.
