In the fall of 2008, the hospital infection control department identified the ED as the unit with the worst hand hygiene in the hospital -- only 56% compliance during monthly audits. This represented a tremendous opportunity for improvement so departmental leadership asked the QI committee to develop a program for hand hygiene improvement. We served as the attending and resident physician champions in a multi-stakeholder group that included nurses, infection prevention personnel, environmental services staff and other healthcare workers.

QIPS TIPS# 17- The Medical Apology: Who and How?

The concept of the medical apology and the surrounding debate over its place in clinical practice arose from accepting a truth that had long gone unacknowledged—Medicine is practiced and delivered by humans, and humans make mistakes; therefore, there are mistakes in medicine. The groundbreaking apology dialogue began in 1994 with Lucian Leape’s article “Error in Medicine.”

TurelliIn this newest edition of the Quality Improvement & Patient Safety (QIPS) Newsletter, we are excited to share with our members the QIPS Section Annual Report and QIPS Section Minutes from the 2012 Scientific Assembly that took place in Denver, CO. Some of the highlights of this year’s Scientific Assembly QIPS events were presentations by Drs. Farley, Schurr and Pines. Dr. Heather Farley gave a summary and QIPS update to the members, while Dr. Jeremiah Schuur, current Chair of the Quality & Performance committee, presented an overview of the committee’s activities. This was followed by a presentation from Jesse Pines, MD, MBA, MSCE, FACEP on “Quality and Patient Safety in Emergency Medicine in the Era of Health Reform.”
